Dandruff Ka Ilaj: A Step-by-Step Plan That Works in 4 Weeks

Short answer: most dandruff improves within two to four weeks when you use an anti-dandruff shampoo correctly. That means an active ingredient such as ketoconazole, zinc pyrithione or selenium sulfide, massaged into the scalp and left on for three to five minutes, two to three times a week. After that, a once-a-week maintenance wash usually keeps it under control.
What dandruff actually is
Dandruff is not dirt and not "dry skin." It's linked to a yeast called Malassezia that lives on everyone's scalp and feeds on scalp oil. In people prone to dandruff, the scalp reacts to the by-products of this yeast with faster skin-cell shedding. The result is flakes that are often slightly greasy and yellowish, along with itching.
That explains three things:
- Antifungal shampoos work because they reduce the yeast.
- Heavy oiling can make dandruff worse, because it feeds the yeast.
- Dandruff comes back if you stop completely, since the yeast is a normal resident.
Before you start: is it really dandruff?
| Looks like | Possibly |
|---|---|
| Greasy, yellowish flakes, itchy, oily scalp | Dandruff |
| Small, white, powdery flakes, tight feeling, worse in winter | Dry scalp. See our dry scalp vs dandruff guide |
| Thick, silvery plaques with clear edges, possibly on elbows too | Psoriasis. See a doctor |
| Redness and greasy scale on eyebrows, nose folds and behind the ears | Seborrheic dermatitis. Read more |
| Patches with broken hairs, especially in children | Fungal infection (tinea). See a doctor |
The 4-week plan
Week 1: start the active
- Choose one shampoo with ketoconazole (1% or 2%), zinc pyrithione, selenium sulfide or piroctone olamine. Our shampoo comparison explains the differences.
- Wash two to three times this week.
- Technique: wet your hair, apply the shampoo to the scalp, massage for a minute, leave it on for three to five minutes, then rinse thoroughly.
- Condition the lengths only, keeping conditioner off the scalp.
- Pause heavy oils on the scalp.
Week 2: keep going and check irritation
- Continue two to three washes a week.
- Mild dryness is common. Use conditioner on your lengths.
- If you notice burning, a rash or swelling, stop and see a doctor.
Week 3: judge progress
- Most people see noticeably fewer flakes and less itch by now.
- If it's only partly better, keep going for one more week.
Week 4: switch to maintenance
- Use the anti-dandruff shampoo once a week, with a gentle shampoo on other days.
- If flakes return, go back to two washes a week.
- Some people find that rotating between two different actives (for example, ketoconazole one week and zinc pyrithione the next) helps.
Common mistakes
- Rinsing immediately. The active needs contact time.
- Using it on the lengths only. Dandruff is on the scalp.
- Oiling heavily the night before every wash. On a dandruff-prone scalp, this can feed the yeast.
- Giving up after one wash.
- Stopping completely once it clears. Maintenance keeps it away.
- Scratching. It worsens inflammation and can cause breakage.
Pakistan-specific tips
- Summer humidity and sweat (Karachi, Lahore in monsoon) often trigger flare-ups. Wash after heavy sweating, and don't leave a sweaty cap or helmet on.
- Hard water doesn't cause dandruff, but it can make shampoo lather poorly. Use enough shampoo and rinse well.
- Winters can bring dry-scalp flaking, which is a different problem. See our winter khushki guide.
- Caps, dupattas and hijabs: wash them regularly, and let your hair dry before covering it.
Diet and dandruff
No specific food has been proven to cause or cure dandruff. A balanced diet, sleep and stress management may help general skin health. Be wary of supplements marketed as a dandruff cure.
What about home remedies?
Some people try lemon, dahi, neem or tea tree oil. The evidence is weak, and lemon in particular can irritate the scalp. Read our home remedies check before you try them.
Quick answers
Can children use anti-dandruff shampoo? Ask a doctor first, because flaky scalps in children can be a fungal infection that needs different treatment.
Will dandruff come back in summer? It often flares with heat and sweat. Go back to two washes a week at the first sign of flakes.
See a doctor if…
- There's no improvement after four weeks of correct use
- Your scalp is very red, swollen, painful, oozing or bleeding
- Thick, silvery plaques or flakes spread to your face, ears or chest
- There are bald or broken-hair patches, especially in a child
- You have a weakened immune system
- Itching keeps you awake at night
Bottom line
Dandruff ka ilaj is mostly about technique: the right active, enough contact time, enough washes per week, and then a maintenance routine. Give it four weeks and you'll know whether you need a doctor.
